I recently interviewed Dr. Myles Spar, a board-certified physician in Internal and Integrative Medicine who is on the clinical faculty UCLA and the University of Arizona about what we should be tracking and what tests to get.

Myles is the author of Oxford University’s Integrative Men’s Health and has a new book coming out in 2020 called, Optimal Men’s Health: Your Guide To Being Healthy In Order To Win. Though his focus is on optimal performance for men, we women need to have these same details and metrics to track our health.

Dr. Spar works with the NBA team and many sports legends and celebrities in the LA area to give them OPTIMAL health. Here are the metrics he tracks for his clients.

These are some of the tests he recommends:

Nuclear Magnetic Resonance, an advanced heart disease panel and an advanced lipid panel that includes total cholesterol, LDL and HDL particle numbers, and LPA metrics.

He also recommends a Coronary Calcium Score test to assess plaque build-up. This is a CT scan and also relatively inexpensive. He says it’s not necessary to have the Carotid Artery calcium test if you get the Coronary Calcium Score test because they both show the same information — how much of your arterial plaque is calcified.


Metabolic testing of your insulin response including fasting insulin, glucose (blood sugar) resistance HOMA-IR numbers, and average blood hemoglobin Al-c.

For Stress, he tests Cortisol levels as well as adrenal and melatonin levels.

He conducts two tests for hormones, a blood test, and a DUTCH or 4-point dried urine test to measure Total Testosterone, Free Testosterone, Estradiol, and DHEA and recommends Total T be 400-950 ng/Dl and Free T 20-50 ng/Dl, Estrogen to be 10-40 ng/Dl. He likes a Saliva test from ZRT Lab as well.

For Bone Health and Body Fat, he recommends a DEXA scan and for men to be under 19% body fat. They have body fat and bone density test. DEXA scans are very inexpensive and DEXA mobiles — little trucks with the scanners in them are often found at health clubs and health events.


He also recommends three specific genetic tests so that you can understand specific sexual health genes. He recommends Stratagene tests from Dan Lynch, the author of Dirty Genes, a book I’ve reviewed in the past. These tests are for the COMT, MTHFR, and APOE genes. Specifically for men, he also recommends a Penile Doppler Ultrasound before going in for a series of GAINSWave treatments and P Shots. He says it’s good to benchmark your before and after GAINSWave treatments, so you can measure the improvements.
